A flag depicting a skeleton with an hour-glass in one hand and cross-bones in the other, standing next to a dart above a heart, from which are falling three drops of blood, is described by Captain Johnson in A General History of the Pyrates. It is the only known source for this flag. ‘ … a Death in it, with an Hourglass in one Hand, and cross Bones in the other a Dart by it, and underneath a Heart dropping three Drops of Blood.’
